Patient Portal

After you have called one of our offices locations and set up your account, we provide you the ability to complete specific forms online via our Patient Portal and submit them to Pathways in a secure and confidential manner.

Access to the Patient Portal requires a User-ID and a password.   A User-ID and “first time use only” password will be provided to you by our Front Office staff.   The portal allows you to view upcoming appointments as well as filling out various forms.

Using the Patient Portal to complete the Client Questionnaire streamlines the upfront process and paperwork needed in order to save you time during the intake process.    The client questionnaire form asks you for basic demographic information, employment and financial information.
